The scope depends entirely on what the audit found, which is why this is priced from rather than fixed. A site with contrast problems and missing alt text is a straightforward job.
One where the navigation cannot be operated without a mouse and the forms give no usable error messages is a much bigger piece of work, because those are structural rather than cosmetic.
The fixes are made within your existing design wherever possible. Accessibility work has a reputation for making sites look worse, and it mostly comes from doing it carelessly. Contrast corrections are the most visible change and they can nearly always be made inside your existing palette rather than by abandoning it.
